The Seventh-Day Adventist Church has a moral and civic duty to protect the children and youth entrusted to its care. The church is committed to providing a safe worship and educational environment to help children and youth learn to love and follow Jesus Christ. According to the Church Manual, Edition 18, pp 168-169, "Everyone involved in work with children who are minors must meet all Church and legal standards and requirements".
The Pennsylvania Conference of SDA has established business accounts with the Department of Human Services, PA State Police Criminal Record repository and Cogent Systems (FBI Fingerprinting). There is no out of pocket expense to the Volunteer. Please be sure to fill out these forms completely.
Background Check Authorization & Disclosure Form The PA Conference has created business accounts with Cogent Systems (FBI Fingerprinting) and the PA State Police Criminal Record repository. Please complete the Background Check Authorization & Disclosure Form so that we may order/register these two clearances for you. If fingerprinting is required, we will contact the volunteer with the registration number as well as a list of fingerprinting locations in the volunteers area. You must have the registration number with you in order to be fingerprinted.
Child Abuse Clearance (Note: This is a new form) - This form must be completed by the volunteer. The form asks for where you have lived and who you have lived with since 1975. If a volunteer was born after 1975, please provide information from Birth - To Present. Please do not neglect to provide this information. Otherwise we will have to return the forms to the volunteer for more information. Please fill out and mail or fax to the PA Conference at the address listed below.
